Assessment and Admissions Process

The admissions office is located in the Blumer Building. Follow campus signs to Partial Hospital/Parking Lot C. Use Partial Hospital entrance at Goddard Building and follow signs to Blumer. Patients can self-refer to the program. Clinicians referring patients, please complete and fax the Ambulatory Referral Form

Frequently Asked Questions

Is lunch provided?
Lunch is not provided, but cafeteria services are available from 11:30 a.m. to 1:30 p.m. You may eat in the cafeteria or in the program space, where there is a room for you to relax and enjoy your lunch. You may also bring your lunch from home. A refrigerator and microwave are available.
May I smoke?
Butler Hospital is a smoke-free, tobacco-free campus. Smoking is not permitted in the buildings or on the grounds. If you smoke or use tobacco products, the hospital will provide medications that may aid in smoking cessation or help to manage nicotine withdrawal symptoms. We appreciate your cooperation with our smoke-free policy.
May I use my phone?
A phone is available for your use during free time and non-group time. Out of respect for patient confidentiality, cell phones are not permitted in the Partial Hospital building.
Is transportation provided?
We do not provide transportation.
Is parking available?
Yes. Ample parking is available and is free of charge. Parking Lot C in the rear of the hospital is the closest one to the partial program, but you may park in any of the hospital's parking lots.
